fdread: Fix logic bug when reading by sector
When reading by sector (because reading a whole track failed), we can accidentally fall into the "should not happen" path, which both (a) emits a spurious error message and (b) fouls up our position accounting going forward. Ensure we do not inappropriately fall into that path.
Avoid obscuring the "short after" message in cases where it happens.

The Software Package Data Exchange (SPDX) group provides a specification to make it easier for automated tools to detect and summarize well known opensource licenses. We are gradually adopting the specification, noting that the tags are considered only advisory and do not, in any way, superceed or replace the license texts.
No functional change intended.

Use NULL instead of 0 for pointers.
The strchr(3) returns a NULL if the character does not appears in the string. The malloc will return NULL if cannot allocate memory.
