KTR: Add CTR() and TR() macros which accept a variable number of arguments.

These can be used in place of the CTRn() macros which require n to match

sys/sys: further adoption of SPDX licensing ID tags.

Mainly focus on files that use BSD 2-Clause license, however the tool I
was using misidentified many licenses so this was mostly a manual - error
prone - task.

The Software Package Data Exchange (SPDX) group provides a specification
to make it easier for automated tools to detect and summarize well known
opensource licenses. We are gradually adopting the specification, noting
that the tags are considered only advisory and do not, in any way,
superceed or replace the license texts.

Expand ktr_mask to be a 64-bit unsigned integer.

The mask does not really need to be updated with atomic operations and
the downside of losing races during transitions is not great (it is
not marked volatile, so those races are pretty wide open as it is).

Right now LOCK_DEBUG is enabled if KTR is on.
This is to support LOCK_LOG_* functionality effectively in debugging
environments but it is overkill because really LOCK_DEBUG should be on
only if (KTR_COMPILE & KTR_LOCK) is true.
Fix this by applying the correct logic.
In this process, move the KTR classes to its own header to reduce
namespace pollution.
