ptrace.2: -width only takes one argument.
swapon.2: <sys/vmparam.h> and <vm/swap_pager.h> weren't being displayed, because .It is for a list item whereas .In is for included files. Also, we want a blank line between <sys/ > headers and the other one.

open(2): Remove O_BENEATH and AT_BENEATH
with the reasoning that the flags did not worked properly, and were not shipped in a release.
O_RESOLVE_BENEATH is kept as useful.

While here, document EIO as a filesystem-independent error for both mount(2) and posix_fadvise(2). EIO was previously only documented for UFS for mount(2).

If the immediately preceding comment is a @(#) format ID marker than the the $Id$ will line up underneath it with no intervening blank lines. Otherwise, an additional blank line is inserted.
