stdio: provide _unlocked variants of fflush, fputc, fputs, fread, fwrite

fflush_unlocked is currently desired in ports by sysutils/metalog, and
redefined as the locked fflush.

mdoc(7) police: markup laundry.

The macro versions of putc() and putchar() are gone.

Document getc_unlocked(), getchar_unlocked(), putc_unlocked(),
and putchar_unlocked().

mdoc(7) police: Fixed abuses of the .Ar and .Em macros.

Consistently mark std(in|out|err) with .Dv, because that's how they
are marked up in stdio(3), and because they are defined expressions
of type "FILE *".

Remove single-space hard sentence breaks. These degrade the quality
of the typeset output, tend to make diffs harder to read and provide
bad examples for new-comers to mdoc.
