Change the way that the queue(3) structures are declared; don't assume that
the type argument to *_HEAD and *_ENTRY is a struct.

Back out all of yesterdays include file changes.

Make a lot of include-files self-contained. I excluded the patches changing
int's to gid_t and uid_t - should I commit these, too?
