Headers in include/sysemu/ are not only related to system *emulation*, they are also used by virtualization. Rename as system/ which is clearer.

mips: Remove support for trap and emulate KVM
This support was limited to the Malta board, drop it. I do not have a machine that can run VZ KVM, so I am assuming that it works for -M malta as well.

In my "build everything" tree, changing hw/hw.h triggers a recompile of some 2600 out of 6600 objects (not counting tests and objects that don't depend on qemu/osdep.h).
The previous commits have left only the declaration of hw_error() in hw/hw.h. This permits dropping most of its inclusions. Touching it now recompiles less than 200 objects.

MIPS KVM trap & emulate guest kernels have a different segment layout compared with traditional MIPS kernels, to allow both the user and kernel code to run from the user address segment without repeatedly trapping to KVM.
QEMU currently supports this layout only for KVM, but its sometimes useful to be able to run these kernels in QEMU on a PC, so enable it for TCG too.
This also paves the way for MIPS KVM VZ support (which uses the normal virtual memory layout) by abstracting whether user mode kernel segments are in use.

Add API for converting physical addresses to KVM guest KSEG0 addresses, and fix the existing API for converting KSEG0 addresses to physical addresses to work in the KVM case. Both have the same sized KSEG0, so it's just a case of fixing the mask.
In KVM trap and emulate mode both the guest kernel and guest userspace execute in useg: Guest User address space: 0x00000000..0x3fffffff Guest Kernel Unmapped: 0x40000000..0x5fffffff Guest Kernel Mapped: 0x60000000..0x7fffffff

This is so that paths remain valid as files are moved.
Instead, files in hw/dataplane are referenced with the relative path. We know they are not going to move to include/, and they are the only include files that are in subdirectories _and_ move.

A few machines need to translate the ELF header addresses into physical addresses. Currently the only possibility is to add a value to the addresses.
This patch replaces the addend argument by and a translation function and an opaque passed to the function. A NULL function does not translate the address.
The patch also convert all machines that have an addend, simplify the PowerPC kernel loading and fix the MIPS kernel loading using this new feature. Other machines may benefit from this feature.
