pc-bios/s390-ccw: Link the netboot code into the main s390-ccw.img binary
We originally built a separate binary for the netboot code since it was considered as experimental and we could not be sure that the necessary SLOF module had been checked out. Time passed, the code proved its usefulness, and the build system nowadays makes sure that the SLOF module is checked out if you have a s390x compiler available for building the s390-ccw bios. So there is no real compelling reason anymore to keep the netboot code in a separate binary. Linking the code together with the main s390-ccw.img will make future enhancements much easier, like supporting more than one boot device.

pc-bios/s390-ccw: Get rid of the the __u* types
The types starting with double underscores have likely been introduced into the s390-ccw bios to be able to re-use structs from the Linux kernel in the past, but the corresponding structs in cio.h have been changed there a long time ago already to not use the variants with the double underscores anymore:
https://git.kernel.org/pub/scm/linux/kernel/git/torvalds/linux.git/diff/drivers/s390/cio/cio.h?id=cd6b4f27b9bb2a
So it would be good to replace these in the s390-ccw bios now, too.

s390-bios: Support for running format-0/1 channel programs
Introduce a library function for executing format-0 and format-1 channel programs and waiting for their completion before continuing execution.
Add cu_type() to channel io library. This will be used to query control unit type which is used to determine if we are booting a virtio device or a real dasd device.

I've run into a compilation error today with the current version of GCC 8:
In file included from s390-ccw.h:49, from main.c:12: cio.h:128:1: error: alignment 1 of 'struct tpi_info' is less than 4 [-Werror=packed-not-aligned] } __attribute__ ((packed)); ^ cc1: all warnings being treated as errors
Since the struct tpi_info contains an element ("struct subchannel_id schid") which is marked as aligned(4), we've got to mark the struct tpi_info as aligned(4), too.

First batch of s390x patches: - 2.11 compat machine - support the new --s390-pgste linker option, making it possible to avoid enabling the global vm.allocate_pgste systl if all pieces are in place - correctly identify some devices as not hotpluggable - clean up some tests and enable them for s390x - wire up the diag288 watchdog in tcg - clean up dependencies on CONFIG_PCI, making it possible to disable it by hand - lots of cleanup in target/s390x/ - fix alignment of the ccw1 structure in the s390-ccw bios - and some more bugfixes
