recompile of some 2700 out of 6600 objects (not counting tests and
objects that don't depend o

In my "build everything" tree, changing migration/vmstate.h triggers a
recompile of some 2700 out of 6600 objects (not counting tests and
objects that don't depend on qemu/osdep.h).

hw/hw.h supposedly includes it for convenience. Several other headers
include it just to get VMStateDescription. The previous commit made
that unnecessary.

Include migration/vmstate.h only where it's still needed. Touching it
now recompiles only some 1600 objects.

Add a linked list of keyboard handlers. Added handlers will go
to the head of the list. Removed handlers will be zapped from

the list. The head of the list will be used for events.

This fixes the keyboard-dead-after-usb-kbd-unplug issue, key events
will be re-routed to the ps/2 kbd instead of being discarded.

HID reorganziation broke the usb tablet in windows xp. The reason is
that xp activates idle before it starts polling, which creates a
chicken-and-egg issue

HID reorganziation broke the usb tablet in windows xp. The reason is
that xp activates idle before it starts polling, which creates a
chicken-and-egg issue: We don't call hid_pointer_poll because there are
no pending events. We don't get any events because the activation code
in hid_pointer_poll is never executed and thus all pointer events are
routed to the PS/2 mouse by qemu.

Fix this by creating a hid_pointer_activate function and call it from
usb-hid when the guest sets the idle state.
