This virtual method was always set to the x86-specific pc_madt_cpu_entry(), even in piix4 which is also used in MIPS. The previous changes use pc_madt_cpu_entry() otherwise, so madt_cpu can be dropped.
Since pc_madt_cpu_entry() is now only used in x86-specific code, the stub in hw/acpi/acpi-x86-stub can be removed as well.

Instead of composing disabled _MAT entry and then later on patching it to enabled for hotpluggbale CPUs in DSDT, set it to enabled at the time _MAT

Instead of composing disabled _MAT entry and then later on patching it to enabled for hotpluggbale CPUs in DSDT, set it to enabled at the time _MAT entry is built.
It will allow to drop usage of packed structures in following patches when build_madt() is switched to use build_append_int_noprefix() API.

Later it will be used for generating x2APIC entries in case of more than 255 CPUs and also would be reused by ARM target when ACPI CPU hotplug is introduced there.
