Move QOM typedefs and add missing includes
Some typedefs and macros are defined after the type check macros. This makes it difficult to automatically replace their definitions with OBJECT_DECLARE_TYPE.
Patch generated using:
$ ./scripts/codeconverter/converter.py -i \ --pattern=QOMStructTypedefSplit $(git grep -l '' -- '*.[ch]')
which will split "typdef struct { ... } TypedefName" declarations.
Followed by:
$ ./scripts/codeconverter/converter.py -i --pattern=MoveSymbols \ $(git grep -l '' -- '*.[ch]')
which will: - move the typedefs and #defines above the type check macros - add missing #include "qom/object.h" lines if necessary

s390x: adapter routes error handling
If the kernel irqchip has been disabled, we don't want the {add,release}_adapter_routes routines to call any kvm_irqchip_* interfaces, as they may rely on an irqchip actually having been created. Just take a quick exit in that case instead. If you are trying to use irqfd without a kernel irqchip, we will fail with an error.
Also initialize routes->gsi[] with -1 in the virtio-ccw handling, to make sure we don't trip over other errors, either. (Nobody else uses the gsi array in that structure.)

core: replace getpagesize() with qemu_real_host_page_size
There are three page size in qemu:
real host page size host page size target page size
All of them have dedicate variable to represent. For the last two, we use the same form in the whole qemu project, while for the first one we use two forms: qemu_real_host_page_size and getpagesize().
qemu_real_host_page_size is defined to be a replacement of getpagesize(), so let it serve the role.

s390x/kvm: Officially require at least kernel 3.15
Since QEMU v2.10, the KVM acceleration does not work on older kernels anymore since the code accidentally requires the KVM_CAP_DEVICE_CTRL capability now - it should have been optional instead. Instead of fixing the bug, we asked in the ChangeLog of QEMU 2.11 - 3.0 that people should speak up if they still need support of QEMU running with KVM on older kernels, but seems like nobody really complained. Thus let's make this official now and turn it into a proper error message, telling the users to use at least kernel 3.15 now.

In my "build everything" tree, changing migration/qemu-file-types.h triggers a recompile of some 2600 out of 6600 objects (not counting tests and objects that don't depend on qemu/osdep.h).
The culprit is again hw/hw.h, which supposedly includes it for convenience.
Include migration/qemu-file-types.h only where it's needed. Touching it now recompiles less than 200 objects.

s390x/flic: factor out injection of floating interrupts
Let the flic device handle it internally. This will allow us to later on store floating interrupts in the flic for the TCG case.
This now also simplifies kvm.c. All that's left is the fallback interface for floating interrupts, which is now triggered directly via the flic in case anything goes wrong.

migration: pre_save return int
Modify the pre_save method on VMStateDescription to return an int rather than void so that it potentially can fail.
Changed zillions of devices to make them return 0; the only case I've made it return non-0 is hw/intc/s390_flic_kvm.c that already had an error_report/return case.
Note: If you add an error exit in your pre_save you must emit an error_report to say why.
