Not all that huge this release, just a number of small fixes and updates:
- software node fixes
- kobject now sends KOBJ_REMOVE when it is removed from sysfs, not when it is removed from memory (which could come much later)
- device link additions and fixes based on testing on more devices
- firmware core cleanups
- other minor changes, full details in the shortlog
All have been in linux-next for a while with no reported issues"

The conversion is actually: - add blank lines and identation in order to identify paragraphs; - fix tables markups; - add some lists markups; - mark literal blocks; - adjust title markups.
At its new index.rst, let's add a :orphan: while this is not linked to the main index.rst file, in order to avoid build warnings.

DRIVER_ATTR is no longer in use, and driver authors should be using DRIVER_ATTR_RW() or DRIVER_ATTR_RO() or DRIVER_ATTR_WO() instead in order to always get the permissions correct. So remove it so that no one can use it anymore.
